Sometime these are also known as lead generation forms, because your intention is to generate leads. If you don’t have one, you should. Using a contact form on your homepage can increase the amount of leads you get. Make it as easy as possible for people to sign up. Don’t make then dig through pages to find out how to sign up. I know this sounds simple, but it’s crucial.

You can keep the form simple. The things you really need are the persons name, phone number and email. A message can be optional. Just make sure you use a strong call-to-action. This means you need to direct the visitor what to do. Don’t just tell them to “submit” the form. Tell to fill out the form for a free-trial workout or whatever it is you want them to do. Then your submit button can say, “Get My Free Workout”. Now, the customer knows exactly what you want them to do and you’ve given them an easy way to do it.
